Medical Services: Primary Care, Pediatrics, Women’s Health/ Prenatal and Medication-Assisted Treatment

Medical Services

The Choptank Community Health System, Inc. (CCHS) is a private, non-profit community health center providing primary health care services in Caroline, Dorchester, Kent, Queen Anne’s and Talbot Counties, and the surrounding areas. As a community health center, CCHS is able to use federal funding to expand current services and develop new programs to meet the healthcare needs in our area.

Primary Care Services 

  • Offered at Bay Hundred, Cambridge, Easton, Denton, Federalsburg, and Goldsboro locations 

  • Providers partner with patients to get to know them and their medical history through regular check-ups and immunizations.  Our providers are also here when you get sick and offer guidance and support in managing chronic conditions. Our goal is to prevent, diagnose and treat conditions in people of all ages

Pediatrics

  • Offered at our Easton and Denton locations 

  • Our pediatricians treat babies, children, and teenagers up to age 21 

Behavioral Health

  • Offered at Bay Hundred, Cambridge, Denton, Easton, Federalsburg, Goldsboro, and Chestertown locations

  • A wide range of behavioral health services available to medical and dental patients.

Women’s Health and Prenatal Care 

  • Offered at our Cambridge, Easton, and Federalsburg locations

  • Women’s Health includes wellness exams including yearly screenings

  • Prenatal care is the health care you get while you are pregnant. It includes your checkups and prenatal testing. Prenatal care can help keep you and your baby healthy.

Medical Assisted Treatment 

  • Offered at our Denton and Federalsburg locations 

  • Help meet recovery needs by providing care to the whole patient with the use of medications in combination with counseling and behavioral health therapies. 

Non-Urgent Follow-Up Visits

Non-urgent follow-up visits may take a week or more to book. Patients’ schedules are always considered, and every effort is made to accommodate individual needs. It is essential, however, that we maintain availability to see patients with urgent problems.

Urgent Care Visits

Patients with urgent care problems such as injuries, infections, sore throats, or similar conditions are usually seen within a day or two. If you feel it is necessary to be seen that day, please call before 10:00 AM. If you have an urgent complaint, we will offer you the earliest appointment available, which may be with a doctor, nurse practitioner, or physician assistant other than your regular primary care physician. Please be flexible. We’re certain that you understand that it is impossible to see patients on a “walk-in” basis.

Unfortunately, it is not always possible to accommodate every single request for a same-day appointment. To see patients as quickly as possible, we may ask you to wait until the following day, or even refer you to Urgent Care for evaluation. Some medical problems, such as acute chest pain or serious injuries cannot be treated within an office setting and necessitate more extensive facilities. In these situations, we may recommend immediate assessment in the Emergency Department.

Labcorp

Facilities in all centers offer basic lab services for all ages. 

Spanish/ Creole Language Lab Services

Lab services are available at our CCHS clinics/LabCorp, with Spanish and Creole interpreters provided at some locations Access to Language Line Solutions is also available at locations. https://www.languageline.com/s/

Appointments

Appointments can be made with any provider by calling the office.  If you are unable to keep a scheduled appointment, please call the office at least 24 hours before the scheduled appointment to reschedule or cancel.

Prescriptions Refills

Please do not wait until you are out of your medication to request a refill.

Requests for refills on prescriptions ordered by your provider can be called in or left with the receptionist. You may pick up your refill prescription or have it called in to your pharmacy within two (2) business days of the request. For routinely prescribed medicines, an appointment is normally required with your provider every six months.

Specialist Referral

If you need a referral from your provider to obtain specialty care, please make requests for non-urgent referrals at least five days before your appointment with the specialist.

Record Requests

All requests for forms to be filled out or medical records copied for other providers, work, school, insurance companies, social services, etc., will be completed within 10 working days of your request.

Hospital Services

If you need to go to the hospital, Choptank Health’s providers have coverage at the University of Maryland Shore Medical Center in Easton.

Office Visits

Office visits for physical exams and new patient evaluations take up more time in the office and therefore are more difficult to work into the provider’s schedule. Visits for routine care are customarily booked within two to three months of the date of your request. We suggest that you plan as far in advance as possible when requesting an appointment. 

Dental Services: Primary Care, Pediatric and Oral Surgery

Within the CCHS mission of providing quality health care for all, the CCHS Oral Health Program is committed to assuring appropriate oral health care for all.

Because of limited capacity and high demand for services, the dental program prioritizes access to care for residents of the Mid-Shore: who are covered by Medical Assistance or are without insurance; are under 20 years old; women who are pregnant; and patients with medical conditions referred by their primary care physician for whom dental care is critical to the management of their health condition. 

The program is also available to individuals experiencing acute dental problems such as severe pain, fever, swelling, or recent injury. Access to emergency care is provided on a walk-in, open access basis.

About our Dental Facilities:

  • Equipped with state of the art dental suites

  • Staffed by licensed dentists, dental hygienists, dental assistants and support staff

  • Focused on children who are uninsured or enrolled in HealthChoice. At this time, due to the tremendous need for oral health services in the area, adult patients will only be seen for dental emergencies or by referral from their medical provider

  • Partners with local community agencies (schools, health departments, Head Start, social services, and many others) to improve the community’s health

  • Collaborates with Shore Health System, and Eastern Shore AHEC to provide hospital-based rehabilitative and restorative services for children. The organizations are members of CROC – Children’s Regional Oral Health Consortium.

Services available:

  • Dental examinations, cleanings, x-rays, dental sealants, fluoride

  • Fillings (including tooth-colored)

  • Oral surgery extractions

  • Root canals

  • Dentures

  • Coordination of hospital-based programs

Community-Based Dental Program

Services may include:

  • a dental screening,

  • cleaning,

  • fluoride treatment (which may possibly be applied two times during the school year),

  • sealants and if needed, referrals for prescriptions and dental emergencies.

Behavioral Health

Choptank Health’s Behavioral Health Services are offered at Bay Hundred, Cambridge, Denton, Easton, Federalsburg, Goldsboro, and Chestertown locations
