School-Based Health Centers on Maryland’s Mid-Shore

Choptank Health’s School-Based Health Centers provide medical, dental, nutrition, and behavioral health services for enrolled students and staff throughout Maryland’s Mid-Shore.

School-Based Health & Dental Centers are a partnership between Choptank Community Health and each of the Mid-Shore county’s public schools. This program allows students to receive medical and dental care right at school.

Care is coordinated with your student's primary care provider and/or primary dentist. Choptank Health will always obtain parent/guardian permission before seeing or treating your student.

School Based Health
Choptank Health provides School-Based Health Centers in partnership with the public schools of Caroline, Kent, Queen Anne's, and Talbot counties, with dental services provided in Dorchester County. 
Any student enrolled in Choptank Health's School-Based Health program can receive care at any location and can receive care regardless of online or in-person attendance. 

Each Choptank Community Health School-Based Health Center is staffed by licensed, advanced practice clinicians, including Certified Nurse Practitioners and Physician Assistants.

School-Based Health Centers function like a typical doctor’s office, with medical providers assessing and diagnosing illness, writing prescriptions, performing lab tests—including for strep, influenza, and COVID—and physicals including routine well visits and sports physicals.

Dental services include screenings, cleanings, sealants, fluoride treatment, and emergency visits, with care coordinated with your child’s primary dentist.

The easiest way to schedule an appointment is by calling the SBHC.  Below are the locations and phone numbers for each of the schools we service. Parents/families can also reach out to the school’s health staff for assistance.
