Choptank Health reopens Tilghman Health Center

Choptank Community Health System is one of 17 Maryland health centers providing affordable, high-quality, comprehensive primary care to people from throughout the local communities, including those without insurance or the ability to pay. Choptank Community Health provides produce prescriptions for students
Choptank Community Health System is providing produce prescriptions to Mid-Shore students through community partnerships with the Avalon Foundation and Caroline County Public Schools Shore Gourmet “Sho Go” Mobile Market. Funding for the project comes from a $25K grant through the School-Based Health Center (SBHC) Food Access Learning Network and Share Our Strength’s No Kid Hungry Campaign.

Choptank Health opens Health Center at White Marsh Elementary School

Choptank Health recently expanded its School-Based Health Center program with the addition of a new health center at White Marsh Elementary School. Choptank Health operates 32 School-Based Health Centers in Caroline, Dorchester, Kent, and Talbot County Public Schools to provide in-person, virtual, curbside well and urgent care, and other services for public-school students enrolled in their school’s wellness center program.
Choptank Health’s new Federalsburg Facility receives town funding

$2.889M for Choptank Health’s new Federalsburg Health Center included in federal funding bill

Choptank Community Health receives No Kid Hungry grant for produce prescriptions
Choptank Community Health System has recently been awarded a $25K grant to provide fresh produce to local students. The initiative is set to begin this spring and also provides students with health education on topics including the importance of a healthy diet and physical activity. Choptank Health’s new Federalsburg Health Center receives County funding

Choptank Health, Preston Motor Group bring mobile primary healthcare unit to Mid-Shore
Choptank Community Health System’s new mobile primary healthcare unit, shown here, is made possible in part through a partnership with Preston Motor Group. The new mobile unit will travel across Mid-Shore communities to meet patients where they are. The unit will additionally provide school-based medical and dental services, and provide a platform for Choptank Health’s migrant program team to visit various farms, agricultural nurseries, and crab houses across the Shore.
